The blocking ask_user_question description did not explain that an answer
item with empty selected and no custom is a user skip, so models re-asked
the same question after the user clicked Skip.
Refs #7134
Replace the literal "Approval policy: ask." runtime-context sentence
with a statement parallel to the never-policy sentence, and refresh the
current-generation fixtures that persist it.

Keyless session, SDK, ACP, and Web snapshots, the subagent-inheritance
expected output, and the Python dynamic-tools snapshot no longer list
exit_plan_mode outside plan mode, and the collapse-timing goldens count one
fewer added tool. In plan-review, the approved exit now removes the tool on
the next step: its header records a change that starts a series and a
tool-removal developer message, which the expanded Web timeline shows.
cordis-inspect-jsdoc records the new tools service signatures. The pwsh
tool-turn pins are edited by hand because pwsh is unavailable for a local
refresh; their only change is the removed schema.
The rendered tool guidance now ends with the end-turn sentence, so every
recorded system prompt and the bilingual package README fence that show it
carry the same text: 83 system-prompt sidecars and 2 README files.
* fix(schedule): keep timing and execution notes out of task titles
* fix(schedule): avoid repeating task title guidance
* fix(schedule): keep title guidance examples in English

The subagent guidance now tells the model to end its turn when only waiting
for delegated results remains, since each result starts a new turn. The read
guidance names sed, head, and tail alongside cat and covers line ranges.
The compaction-output-reserve and compaction-summary-headroom context windows
grow by 40 tokens to keep their triggers between the history and its
condensed replacement.
Each recorded runtime-context snapshot with optional contexts gains the
tokenized checkout and Web GUI URL lines, matching the refresh output of
the code-language scenario, where only that message changed. The
minimal-preset snapshot suppresses optional context and stays unchanged.
The harness checkout path and Web GUI URL now reach the model as runtime
context from the new dsh-harness-dev-context plugin, so a process started
on another port, checkout, or installation no longer rewrites a Session's
logged system prompt. A Session whose prompt already embeds the earlier
value-bearing paragraphs keeps them verbatim through a host-only session
projection.
dsh-host-webserver gains WebServer.origin and bindUrlHost(); web-app keeps
only DSH_WEB_URL, the startup URL, and the browser handoff, and its root
declares the webStartup service type. app-boot drops
addHarnessSourceSection and HARNESS_SOURCE_SECTION.
Preserve the persistent runtime package description with master's alpha.2 version and locale exports. Refresh the two PTC prompt snapshots for the current plan-tool result type.
Validation: full build, 434 focused tests (one skipped), two built-runtime tests, two selected snapshots, package metadata, and all 43 doc-sync checks passed.
Choosing "Request changes" on a plan review rejected the question, and
exit_plan_mode turned that into a failed tool result asking the model to
stop. The loop then spent one more model request only to have the model
wait. A dismissed review now returns { approved: false } and concludes the
turn, so the user's next message starts the following request.
