`turn/*`, `step/*`, `system/message`, `user/message`, `assistant/message`, `assistant/attempt`, and `tool/*` are durable session events; the rest are live extension points across three domains. `agent/assistant-stream` publishes process-local start, transient chunk, and end frames. The loop commits the complete compact stream as one message or log-only attempt before a committed end frame, and the Web Session-follow adapter is the live event's only remote consumer. `agent/pre-step`, `agent/request`, `llm/stream`, and the three `tools/*` events are waterfalls, whose listeners must call `next()` to delegate; `agent/turn-stopping` is serial and has no `next()`.
The factory enters SessionStore and completes `session/created` before setup; `agent/created` follows setup. Disposal commits closing events before removal.
One inbox feeds the driver; injected context waits for a waking message. AgentLoop’s durable `inbox` projection exposes pending input without live Agents.
`agent/pre-step` decides the accepted input. Listeners may rewrite or reject claimed messages; a rejected or empty first claim closes a durable turn without a step. An enter decision may set `startsRequestSeries`: the loop logs a fresh `request/header` (reason `series`, or `change` with `startsSeries: true` when the envelope also changed). Wrapping listeners preserve that declaration with `{ ...decision, messages }`. After assembly and `step/start`, `agent/request` and `prepareCall()` resolve the actual route before the system prompt and accepted users are committed; cancellation during either async phase commits neither. Prepared call capabilities govern prompt admission, not preceding `request/context`. Each attempt refreshes registered runtime facts, reconciles the bound prompt, admits users once, and restores removed runtime context, logs header/context as needed, and freezes the request before streaming the bound call. Retries do not repeat assembly or `agent/pre-step`. Surface replacements and image-offload decisions after attachment start a new request series, including during the first resumed pre-step; unchanged resume continues the series. The first admitted step reserves the system head before user messages even for an empty prompt (no wire message). The prompt travels only as `system/message` history: an empty rendering clears all active system nodes, leaving no old prompt model-visible; capable routes append non-empty updates after cached history, including supported tool updates; incapable routes and new request series consolidate non-empty prompt text at the first system node, with logged empty replacements for non-empty later system nodes ([decision](../.agents/notes/implemented/architecture/2026-09-02-system-prompt-as-surface-node.md); [decision rule](../packages/core/agent-loop/README.md#understand-the-implementation)).

`turn/*`、`step/*`、`system/message`、`user/message`、`assistant/message`、`assistant/attempt` 和 `tool/*` 是持久会话事件;其余是分属三个事件域的实时扩展点。 `agent/pre-step`、`agent/request`、`llm/stream` 和三个 `tools/*` 事件是 waterfall(瀑布式事件),其监听器必须调用 `next()` 才能委托下去;`agent/turn-stopping` 是 serial 事件,没有 `next()`。

The `setup` callback (`AgentSetup`) receives `(agentCtx, agent)` while both ids are still unpublished: the context owns scoped registrations, while the explicit Agent supplies the exact child Session without a reverse property on the Context. Everything registered through `agentCtx` exists before `agent/created` and the first prompt assembly. Setup may return a synchronous commit invoked immediately before publication; a setup rejection, commit throw, or owner disposal rolls the transaction back without publishing either id.

The `setup` callback (`AgentSetup`) receives `(agentCtx, agent)` after Session store entry and completed `session/created`, before Agent registration: the context owns scoped registrations, while the explicit Agent supplies the exact child Session without a reverse property on the Context. Everything registered through `agentCtx` exists before `agent/created` and the first prompt assembly. Setup may return a synchronous commit invoked immediately before publication; a setup rejection, commit throw, or owner disposal unwinds the transaction, removes the Session, and preserves restored pending input and already stored events.
`AgentFactory` is the creation interface behind the registry: the loop registers its factory via `ctx.agents.setFactory()`, so consumers use `ctx.agents` without depending on the concrete loop package. A runtime child creator sets `options.parentAgent`; the registry passes the options and caller Context to the factory without deriving one from the other. The exact `create`/`resume` signatures and rollback contracts are in the [generated section](#ctxagents--agentregistry) below.

# Store-owned Session writes follow publication
English | [中文](guide.zh.md)
## Change
`Session.append()` and `appendPluginRecord()` on store-owned instances reject writes until the instance enters a `SessionStore` and its synchronous `session/created` dispatch completes, and after it leaves. Store ownership starts at `prepare()` or `enter()`. Creation listeners cannot append; a failed announcement leaves writes disabled. Independent `Session.create()` and `Session.fromRestore()` instances remain writable without notifying store observers. Constructor, restore, and fork history still load normally. The Agent factory completes `session/created` before invoking setup; `agent/created` still follows setup. During `session/created`, `ctx.agents.get(session.id)` now returns `undefined`. Listeners registered inside setup miss their own Session’s creation announcement. Setup events now reach live Session subscribers as well as persisted history.
A failed setup retains constructor history and setup events already written to persistence. A failed seeded creation, including a fork, can therefore leave a stored Session; its identity is no longer available for a new creation.
Permission defaults are initialized during `agent/created`, so creating a bare Session no longer pins them. `dsh-permission-presets` now requires the `agents` service as well as its existing dependencies.
## Migration
1. Use `ctx.sessions.create()` for a writable Session owned by the calling fiber. For explicit lifecycle ownership, call `prepare()`, `enter()`, and `announce()` before appending; retain the returned disposer until all closing events finish.
2. Keep `Session.create()` and `Session.fromRestore()` for independent log construction. Build history before handing the instance to `enter()`, or pass it as the seed to `prepare()`. Removal does not restore independent write access. Cold durable writes use the persistence write handle.
3. Move writes and Agent-dependent work out of `session/created` listeners. Use setup or `agent/created` for writes; use the supplied Agent in setup and the `agent/created` payload instead of looking up an Agent during Session announcement.
4. Replace setup-installed observers of their own `session/created` with `agent/created` listeners. Keep Session creation observers in a scope installed before factory creation starts.
5. After failed creation with persisted history, retry with `ctx.agents.resume({ resumeSessionId })` instead of `create()` on the same id. Retain the stored history for recovery.
6. Custom profiles mounting `dsh-permission-presets` must also mount `dsh-agent`, the provider of `agents`, and await permission plugin activation before creating Agents.
7. Verify that creation observers see the expected registry state, permission initialization completes, failed creation can resume its stored history, and rejected appends leave history unchanged.

[English](guide.md) |
Store 从 `prepare()` 或 `enter()` 开始接管实例。 独立的 `Session.create()` 和 `Session.fromRestore()` 实例仍可写入,且不通知 Store 观察者。 Agent 工厂在调用 setup 前完成 `session/created`,`agent/created` 仍在 setup 后执行。在 `session/created` 期间,`ctx.agents.get(session.id)` 现在返回 `undefined`。
2. 独立日志构造继续使用 `Session.create()` 和 `Session.fromRestore()`。
4. 将 setup 内监听所属 Session 的 `session/created` 的代码改为监听 `agent/created`。
5. 创建失败但已持久化历史时,使用 `ctx.agents.resume({ resumeSessionId })` 重试,不要对同一 id 再次调用 `create()`;保留存储历史用于恢复。

### Creation and teardown
Creation is one rollback-covered transaction: construct a private session, concrete agent, and scoped context; await optional setup; enter both registries; announce `session/created`; await serial `agent/created` listeners; then release queued input. A caller creating a runtime child sets `options.parentAgent`; the caller Context separately owns the transaction and live handle. Setup, commit, or listener failure and owner disposal roll back the prepared resources. Announcements already delivered remain observable and receive matching disposal notifications. Teardown stops and drains the driver, unwinds the scope, closes the session's write path, detaches the agent, then detaches the session. Every detach binds the exact entered object, so a stale disposer cannot remove a later same-id replacement.
Creation is one rollback-covered transaction: construct a session, concrete agent, and scoped context; store constructor history, enter the SessionStore, and complete `session/created`; await optional setup and its commit; enter the AgentRegistry; await serial `agent/created` listeners; then release queued input. A caller creating a runtime child sets `options.parentAgent`; the caller Context separately owns the transaction and live handle. Setup, commit, or listener failure and owner disposal roll back the prepared resources. The Session is writable during setup, and every removal emits `session/disposed`, including setup failure before Agent creation is announced. Failed creation preserves restored inbox input and any events already stored.
The loop awaits inbox and turn-boundary registration before publishing its factory and passes those handles into synchronous Agent construction. During factory unload, global registrations may disappear before teardown finishes; retained handles remain readable through scope cleanup, including cancellation from scoped abort listeners. Caller unload, factory unload, and handle disposal await the same teardown.
### Persistence integration
The loop is the production acquisition point for session write handles. When `ctx.sessionPersistence` is mounted, `create`/`createAgent` call `persistence.create(header)` — storing the durable identity and taking write ownership before publication — and append the constructor seed through the handle; `resume` calls `persistence.open(id, 'write')` first (excluding a concurrent resume of the same id), reads the physically valid log through the handle, and appends `interruptedTurnClosers` for a log crashed mid-turn as an ordinary batch — semantic crash repair is the agent layer's job, not a storage entry point. Immediately before publication, `appendUnstoredSuffix` stores any events appended during the setup window (seed markers, delegation policy records), which never re-emit through `session/event`. Once published, the mounted backend routes the session's `session/event` batches, `session/flush` barriers, and `session/disposed` retirement into the active write handle by session id; the loop touches storage only through the handle it owns. With the backend still mounted, teardown closes the handle after the driver commits its closing events; close drains the routed buffer and releases write ownership. Without a backend, sessions are memory-only and nothing else changes.

### Session pinning and blank reuse
Mounting pins every live and future session: a genuinely fresh session gains the configured default preset and both knob facts, while seeded or partially initialized sessions keep their effective knob values and gain only missing durable facts. The projection-owned seed marker makes this decision from the same incremental state as the knob values. The synchronous creation listener is installed before projection registration starts. While its reader is not ready, Session creation fails and Agent creation rolls back; callers must await plugin activation before creating or restoring sessions. A stored `auto` identity fails publication when the Auto integration is absent or rejects admission; the service neither rewrites it nor silently derives Full access.
Mounting pins the Sessions of every live and future Agent: a genuinely fresh session gains the configured default preset and both knob facts, while seeded or partially initialized sessions keep their effective knob values and gain only missing durable facts. The projection-owned seed marker makes this decision from the same incremental state as the knob values. The service installs its `agent/created` listener before starting projection registration, so creation during activation fails instead of skipping pinning. `prepend: true` places pinning before ordinary creation observers, which may read the recorded permission facts. A later listener using `prepend` can run earlier and must not assume pinning has finished. While its reader is not ready, Agent creation rolls back; callers must await plugin activation before creating or restoring Agents. Creating a Session without an Agent does not initialize permissions.
