A reviewer denial now returns the tools pipeline's ask decision when the Session's approval policy is ask, so the user can approve, reject, or cancel the call. Selecting Auto writes the ask policy; a recorded Auto selection still matches never, which delegated children pin, so their denials stay final. A reviewer failure now fails the call with its specific error instead of presenting it as an Auto review denial.
