Turtle bcc5e79c5c fix(auto-review): keep reviewer input append-only for prefix caching ...

Project instructions loaded mid-session were inserted into a section ahead of
the filtered history, so every nested AGENTS.md load changed the review prefix
and invalidated the provider's cached history. Render them in surface order
inside FILTERED_HISTORY with their constraint role, so each review's text
extends the previous review's text through its last history entry.
