Yichen Jiang d7f96a2731 docs(session): add the session.create blank reuse upgrade guide ...

session.create now reuses a named Session only while it is a reusable blank,
runs the requested or default preset, and creates a new Session otherwise.
Record the migration for JSON-RPC clients and Client plugins that name a
sessionId, including the removed session/writer-held result and the
existingPreset detail.
