refactor(client): bind context menus at their Slot consumers
...
Merge master and preserve pinned-row visibility. Remove redundant Chat and tool-tree menu forwarding, unify trigger names, and bind file targets at the consuming Slot. Address CI static and coverage gaps without changing upstream scroll assertions.

Show downscale facts and localize the read_image size line
...
Name the original size when the store downscaled the image, keep repeated
whitespace in the path, localize the Chinese size line, and gate the card
on the envelope separately from collecting extra text.

...
The expanded read_image row no longer repeats the file name above the
image or prints the raw <path>/<type>/<content> envelope below it. It now
shows the full recorded path and each image's WxHpx and byte size in the
small code font; text appended by post-execute hooks stays visible.

fix(ui-tool): keep bash rows expandable with Host-accepted escalation fields
...
The Bash tool accepts sandbox_permissions equal to the current mode with a
blank justification, and a blank justification alone. The terminal card
rejected both, leaving successful commands without an expandable body.
Escalation validity depends on the Session sandbox mode, which only the Host
knows; a rejected call already settles as an error result.
